Fix notes:

Lightweight hiss removal using iZotope RX2 denoiser.

Speed adjustment needed, running too fast.  Sides 1 & 3 adjusted with factor of 97.5%, but side 2 of tape running amazingly fast: factor of 90%.  After adjustments, still some indications that speed may be a little slippy.
At 4:04 in Baby Can Dance there's a sudden deceleration (and more so four seconds later).  Speed corrected whole section with factor of 106%, but could only do at single setting, as no accelerate/decelerate tool available.

Tape flip between Go Now and Betty Wrong - seamless.
Tape flip between Baby Can Dance and You Belong In Rock And Roll - seamless.
Tape flip between Baby Universal and Heaven's In Here - seamless.

Small gaps removed in encore breaks at ends of Heaven's In Here and You Can't Talk - crowd noise only.

Ticking noise on early songs on the source tape - removed using Goldwave Pop/Click tool on default setting (for whole recording).
Dehiss using iZotope RX2 denoiser tool, two passes @ 18.4 dB noise reduction, smoothing @ 8.0
iZotope RX2 dehum tool then used, 50Hz with harmonics
Goldwave parametric equaliser then used, with 150Hz and 400Hz both set to -3dB

Tape flip during The Width Of A Circle, at around 4:44.  May have lost a few seconds of jam, but can't hear it, if so.
Small drop-out in crowd noise at the very end of Rock'n'Roll Suicide - left as is.

Three short cut-outs during The Jean Genie at 6:24, 6:27 and 7:11 (crowd noise).  All removed with negligible impact.
Gap between Breaking Glass and Fame.  Removed, but start of Fame is clipped.  Coincides with tape flip.
Gap and restart during Soul Love @ 1:07.  Repaired, seamless.
Gap removed @ 3 seconds in Star - minimal impact.
Gap in crowd noise @ 3:05 in Art Decade.  Removed, seamless.

Tape flip between Alabama Song and Station To Station - start of latter clipped.
Tape flip between Stay and TVC 15 - end of Stay is missing.  Fade out/in.

Whoosh sound addressed in Goldwave using parametric equaliser:

Cut in crowd noise between Golden Years and Fashion.  Tidied up.
Gap removed between Red Sails and Breaking Glass (probably former tape flip).  Crowd noise only.
Volume boost, Breaking Glass - China Girl: 25% to bring in line.
Couple of minor sound fluctuations in crowd noise between Cat People and China Girl - removed.
Gap in tape @ 4:07 in China Girl (perhaps an old tape flip).  Removed with fade-out/in, but some music missing.

Tape flip between China Girl and Scary Monsters - crowd noise.
Small gap in crowd noise on fade of White Light/White Heat removed.

After Cracked Actor, appears that a radio advert or something similar played over the PA.  Left alone.
Gap removed @ 4:41 in Fame.  Minor loss of music.

Have seen listing indicating that TVC15 was between Fame and Star, but absolutely no evidence here.
