Seamless fix of shnid 11896

I reversed the fade at the end of CD 2 to match the volume of a splice point at the
beginning of CD 3, then deleted the redundant material to make the transition
seamless.  SBEs in CD 3 were fixed using shntool; the only other SBE comes at the
end of CD 1, which didn't seem necessary to fix.  All SHNs were converted to FLAC.
